Cadet Forms

I won't bring the photocopied cadet forms tomorrow because they won't be safe. :D
Here's a copy:

OJ: Batch 2012, one member in your class has been given a note and a copy of the Cadet Form for next year. Additional notes: Only black ink should be used & you should put your whole name in both the left side and the right side. Even Medic applicants will have to fill up the form. You may submit your forms on Monday or next year. If you will submit it on Monday, there is no need for a 1x1 picture. For next year, the picture you should put there should be recent (over the summer taken) & dapat red yung background. Thank you.-gm-

I'll try finding a pdf version tho.

This is due on Monday or Next school year. Take note that you need a fingerprint so you need a stamp pad. :D
