Fill up the necessary information, leaving , ,

empty. Put IV or FOURTH in the year level and put CADET in Rank. The school year will be 2011-2012. Please fill the form out, writing legibly in ALL CAPS and keeping the paper uncrumpled and unfolded. There should be no erasures Please pass your forms to Dave Mendoza, Lance Calub, Orville Feliciano, OJ Acopiado or Kim Palomique on/before Monday, March 21. Thank you.

I can't scan the form. D: It's on a long bond paper. Shall try photocopying tomorrow. BUT I think I shouldn't because you'll only crumple/fold/lose it. o.o But if there's no other choice, i'll just photocopy. I'll also check other sections if they upload. If someone does, i'll just post the link and then download and print it on your own. That way, siguradong hindi crumpled/folded ung form niyo. Para rin pag nagkamali kayo ng sulat, you can just reprint.

Batch 2012, one member in your class has been given a note and a copy of the Cadet Form for next year. Additional notes: Only black ink should be used & you should put your whole name in both the left side and the right side. Even Medic applicants will have to fill up the form. You may submit your forms on Monday or next year. If you will submit it on Monday, there is no need for a 1x1 picture. For next year, the picture you should put there should be recent (over the summer taken) & dapat red yung background. Thank you.-gm- (sorry sa mga hindi nakatanggap
